At Smile Haven, after scheduling an appointment with us, you will receive a link to access your secure chart to complete your new patient forms prior to your appointment. Our patient-friendly platform includes plenty of visuals and guides you through the questionnaire smoothly. You can also upload insurance documents and enter additional family members.
During your initial visit, we will take all necessary X-rays and photos to assess your oral health thoroughly. Dr George will then perform a comprehensive exam and discuss her findings with you. Together, we’ll develop a personalized treatment plan tailored to your specific needs and goals.
If any treatment needs are identified, our team will present you with transparent options and assist you in scheduling follow-up appointments. As your visit concludes, you’ll have a summary of your oral health status and what can be done to improve it.
We accept most dental PPO insurance plans and are happy to process the claim and track it on your behalf. As a courtesy to you, our friendly staff will contact your carrier and verify your plan details, including coverage amounts and covered procedures.
We will need a copy of your insurance card to contact your carrier directly for a benefits breakdown. Every policy is different, and the plan details are typically negotiated by your employer. Our insurance coordinators will work closely with your insurance carrier to help you maximize the coverage and benefits included in your unique plan.
