Smile Haven offers an in-office dental wellness plan for our patients who do not currently have dental coverage. It allows our patients to receive optimal dental care while regularly maintaining their oral health. This plan provides easy, comprehensive and affordable dental care benefits and eliminates the worry of unexpected expenses or undisclosed fees.
With our Wellness Plan, you can receive the necessary care whenever you need it.

The path to membership is short and easy! Complete the enrollment application and return the form to our practice. You do not have to deal with the headaches of insurance, such as denial of claims or pre-approvals.

Important Disclosures

Your membership is valid for 12 months from your date of enrollment.
This plan is not insurance and is not intended to replace dental insurance. Our membership plans are provided only to uninsured patients of our practice.
Membership plans are not (i) insurance, (ii) qualified health plans under the Affordable Care Act, (iii) pre-payment for future services, or (iv) payment for access to discounted services. Membership plans are direct payment arrangements between plan members and Providers for services, products and care provided. Our patients elect to make an annual subscription payment for preventive care versus paying at the time of each service.
Membership plan payments must be current for the patient to receive the benefits included with their membership plan. The savings offered in the membership plan cannot be combined with any other offers or discounts.
Payment for treatment not included in the membership plan is due at the time of service.
